WAIVERS

All riders must sign a waiver, unless you have an current and up to date USABMX membership.

Please note the following information when completing the form. Select "NJ- Central Jersey BMX" from the drop down. The date of the birthday party you are attending is to be used for the date of the event. The waiver must be completed and signed for the rider to be able to participate. Waiver can be completed in person the day of if the parent or guardian will be present.
